The following forms are available in Adobe PDF format , to be printed and completed by hand. Completed forms can be handed in to any collection point (branch office) (when open) or posted directly to the main office.


Membership Application Form (main) (A4, 2 sides). This form is used to provide the minimum information needed from new applicants. Please print, complete and return to your nearest ROMCUL branch/collection-point.	PDF (41kb)
Supplementary Form: volunteering (A4, 1 side). ROMCUL relies very much upon volunteers (all of whom are members) to carry out many of its essential operations. This form lists some of these areas. If you can spare some time to help in running the credit union, please print this form, complete it and return it to your nearest ROMCUL branch/collection-point (not necessarily when joining). Supplementary Form: additional information (A4, 2 sides). ROMCUL relies very much upon information provided by its members to ensure that it is meeting their needs as far as possible. This form is used to gather market research information, etc., anonymously. You will be helping us immensely by completing and returning this form at any time.	PDF (32kb)

Loan Application Form A loan application can only be processed after a fully completed and signed form has been received with a full set of supporting evidence.	PDF (114kb)

Share Withdrawal Form A share withdrawal application can only be processed after a fully completed and signed form has been received at least 3 working days before the shares are required.	PDF (65kb)
